ABSTRACT

Introducción: Una nueva intervención de salud pública, como la introducción de una vacuna, implica el monitoreo de indicadores que aseguren una intervención efectiva, y que exista la posibilidad de cuantificar sus beneficios. Obtener estimaciones precisas del impacto de una intervención de salud se considera un desafío importante. Objetivos: Estimar el impacto causal de uno de los productos líderes del Instituto Finlay de La Habana: la vacuna cubana registrada y comercializada VA-MENGOC-BC®. Métodos: Se seleccionaron datos en los anuarios estadísticos de salud desde 2009 hasta 2017. Se usaron como variable de interés, la tasa de incidencia de la enfermedad meningocócica en Cuba ( 100 000 habitantes y un conjunto de covariables que no se afectaron por la intervención: tasas de incidencia de fiebre tifoidea, de incidencia de tuberculosis, de mortalidad por enfermedades infecciosas (parasitarias e intestinales) todas ( 100 000 habitantes. Se consideró 1989 como año de la intervención. Se aplicó el método bayesiano de series temporales estructurales, que evaluó el impacto causal de la vacunación sostenida con VA-MENGOC-BC® desde 1989 hasta el presente. Resultados: Se estimó un impacto causal acumulativo significativo en la reducción de la incidencia de la enfermedad meningocócica. Se verificó que se produjo una disminución de la enfermedad en 97,2 %. Conclusiones: La aplicación del método de series de tiempo estructural bayesina para estimar el impacto de la vacuna VA-MENGOC-BC®, constituyó una herramienta novedosa para evaluar el contrafactual. Se proporcionó una apreciación del impacto de la vacunación con VA-MENGOC-BC®, una vacuna implementada y reconocida a nivel mundial.


Introduction: A new public health intervention, such as the introduction of a vaccine, implies monitoring the indicators that guarantee its effectiveness, and the possibility of quantifying its benefits. Obtaining accurate estimates of the impact of a health intervention is considered a major challenge. Objective: To estimate the causal impact of one of the leading products of the Finlay Institute in Havana: the registered and marketed Cuban vaccine VA-MENGOC-BC®. Methods: Data from the health statistics yearbooks from 2009 to 2017 were selected. The incidence rate of the meningococcal disease in Cuba per 100 000 population and a set of co-variables that were not affected by the intervention, such as incidence rate of typhoid fever, tuberculosis, and fatality cases due to infectious diseases (parasitic or intestinal) per 100 000 population were used as variables of interest. The intervention year was 1989. The Bayesian structural time series model was applied to evaluate the causal impact of the continued vaccination with VA-MENGOC-BC® from 1989 to date. Results: A significant cumulative causal impact in reducing the incidence of meningococcal disease was estimated. A decrease of 97.2% in the disease was verified. Conclusions: The application of the Bayesian structural time series model to estimate the impact of the vaccine VA-MENGOC-BC® was a novel tool to estimate the counterfactual. It was provided an estimate of the impact of the vaccination with VA-MENGOC-BC®, an implemented and globally well-known vaccine.

ABSTRACT

To study the etiology of papilloedema in north indian population. Our hospital based retrospective study enrolled 50 patients of papilloedema, who fullfilled our inclusion criteria. Demographic details, chief complains and detailed history was taken from all the patients. All the patients underwent blood pressure measurement by a mercury sphygmomanometer, Visual Acuity testing, Slitlamp Biomicroscopy, IOP measurement, Fundus examination. Fundus photographs were obtained using a fundus camera. Other investigations included B-scan, MRI, MRV, CT and required blood investigations were done wherever necessary. Although papilloedema occured in a vast variety of age group but maximum patients (32%) were from the age group of >18-31 years and males (64%) were more affected than females according to our study. 66% of the patients had good vision with papilloedema. In our study all the patients had bilateral papilloedema and 54% of patients were diagnosed with early papilloedema. Idiopathic Intracranial Hypertension was the main etiology for papilloedema. In this study we concluded IIH (a diagnosis of exclusion) as the most common underlying etiology for papilloedema

ABSTRACT

Objective: To investigate the applicability of the spray-dried microspheres of vinyl pyrrolidone/vinyl acetate copolymer VA64-Soluplus for inclusion of cinnamon oil (CO) and compare with traditional inclusion technology of β-cyclodextrin. Methods: HPLC was used to determine the encapsulation rate of inclusion complex. Transmission electron microscopy (TEM), scanning electron microscopy (SEM), differential scanning calorimetry (DSC), and X-ray powder diffraction (XRD) were used to characterize the inclusion complex; The dissolution and stability of the inclusion complex was investigated by in vitro release test and accelerated stability test; The pharmacokinetic and analgesic efficacy tests were used to examine the bioavailability and efficacy of the inclusion complex. Results: The encapsulation rate of microsphere inclusion complex and β-cyclodextrin inclusion complex was (98.38 ± 0.30)% and (86.51 ± 0.52)%, respectively. Observation of the inclusion complex under TEM showed a uniform spherical-like structure with uniform dispersion; Observation under SEM showed that the inclusion complex was spherical with a concave surface; The endothermic peak of volatile oil of cinnamon in DSC and the diffraction peak in XRD disappeared. The cinnamon volatile oil was dispersed in theinclusion complex in the form of non-aggregation; The cumulative release rates of cinnamon volatile oil, microsphere inclusion complex and β-cyclodextrin inclusion complex in in vitro dissolution experiments were 97.05%, 93.36% and 80.26%, respectively; Accelerated stability test at 60 ℃ showed that the loss rate of volatile oil of microsphere inclusion complex was significantly lower than that of cinnamon volatile oil and β-cyclodextrin inclusion complex; Pharmacokinetics showed that the AUC0-∞ of cinnamon essential oil, microsphere inclusion complex and β-cyclodextrin inclusion complex were basically the same; Pharmacodynamics showed that the analgesic rates of cinnamon volatile oil in the three groups were 53.0%, 47.5% and 21.1%, respectively. Conclusion: The stability of cinnamon volatile oil was enhanced by the combination of spray-dried microspheres of vinyl pyrrolidone/vinyl acetate copolymer VA64-Soluplus. The in vitro release, bioavailability and analgesic efficacy of microsphere were basically consistent with the volatile oil of cinnamon volatile oil, and it was superior to the inclusion compound of β-cyclodextrin. The vinyl pyrrolidone/vinyl acetate copolymer VA64-Soluplus microsphere inclusion compound had better water solubility. This study provides a new method for the inclusion of volatile oil.

ABSTRACT

Abstract: Optic Neuritis (ON) is a condition caused by the inflammation of the optic nerve, causing diminished visual acuity and ocular pain. It is tightly related to Multiple Sclerosis (MS), often being the first demyelinating event. There is a 31% risk of recurrence during the first 10 years after the diagnosis, and 48% of the patients end up being diagnosed with Multiple Sclerosis1. Imaging studies like brain MRI (Magnetic Resonance Imaging) have a critical role in the diagnosis and prognosis of ON, as well as in the recognition of MS.2 The patient is a 11-year-old girl with recurrent ON and past medical history of Acute Disseminated Encephalomyelitis (ADEM) when she was 5 years old. ON diagnosis was based on clinical findings as well as on ophtalmologic, electrophysiologic and imaging studies. The recurrent episodes of ON improved after the use of high dose steroids. Recurrences were observed after titration of the dose, but remission was achieved after adjustment of treatment. During one of the recurrent episodes, blood work was performed to evaluate possible underlying infectious, demyelinating or autoinmmune process. Anti-MOG antibodies were found positive. The patient at the moment is not presenting with any other criteria suggesting MS or Optic Neuromyelitis, but long term follow up is adviced. ABSTRACT

La meningitis meningocóccica continua siendo un problema de salud en diferentes países y para la prevención de esta enfermedad se han obtenido diferentes vacunas. La vacuna VA-MENGOC-BC® ha constituido ser eficaz y segura en la prevención de la meningitis meningocóccica contra los serogrupos B y C. Esta ha demostrado buena estabilidad en el tiempo sin cambiar su calidad como producto; fue conservada a estante durante 24 y 36 meses a temperaturas de 4 a 8 °C. Se evaluó su posible potencial toxicológico a través de un estudio de tolerancia local en ratas Sprague Dawley para extender su vida útil. Los animales inmunizados se observaron diariamente para evaluar síntomas locales y sistémicos de toxicidad. Se realizaron evaluaciones del peso corporal, consumo de agua y alimento, termometría, musculometría e irritabilidad dérmica por el método de Draize. Se realizaron estudios anatomopatológicos periódicos para observar posibles efectos adversos. No se observaron síntomas de toxicidad ni muertes. No se encontraron diferencias entre los grupos experimentales en cuanto al peso corporal, el consumo de agua y de alimentos, no se evidenció fiebre, ni irritabilidad local. Anatomopatológicamente a nivel del punto de inoculación se observaron procesos granulomatosos de tipo macrofágicos característicos en las vacunas que contienen hidróxido de aluminio. Estos resultados permitieron concluir que la vacuna VA-MENGOC-BC® que permaneció en estante durante 24 y 36 meses no evidenció efectos adversos locales, ni sistémicos en las ratas(AU)


Meningococcal meningitis continues to be a health problem in different countries and different vaccines have been obtained for the prevention of this disease. VA-MENGOC-BC® vaccine has been effective and safe in the prevention of meningococcal meningitis against serogroups B and C. This has shown good stability over time without changing its quality as a product; it was stored on a shelf for 24 and 36 months at temperatures of 4 to 8 °C. Their possible toxicological potential was evaluated through a local tolerance study in Sprague Dawley rats. Immunized animals were observed daily to evaluate local and systemic toxicity symptoms. Body weight, water and feed intake, thermometry, musculometry were performed and dermal irritability by the Draize method. Anatomopathological studies to observe possible adverse effects were made. No symptoms of toxicity or deaths were observed. No differences were found between the experimental groups in terms of body weight, water and food consumption, no fever or local irritability was evident. Anatomopathologically no lesions of diagnostic value were observed, at the site of inoculation, granulomatous processes of macrophagic type characteristic in vaccines containing aluminum hydroxide were observed. These results allowed us to conclude that the VA-MENGOC-BC® vaccine that remained on the shelf for 24 and 36 months did not show any local or systemic effects in rats(AU)

ABSTRACT

Lutzomyia longipalpis es el principal v ector de una importante enfermedad desatendida en América. La diversidad genética de este vector se estimó en la población colectada en dos áreas geográficas separadas por hasta 37 km. Analizamos la secuencia CB3­PDR / N1N­PDR de 22 individuos obte­ niendo un parámetro de: h = 0.43 y π = 0.0017 (Bona), h = 0.89, π = 0.004 (El Limón) con una dife­ renciación genética de kst = 0.03; p> 0.05 entre ellos. Ocho haplotipos fueron detectados, de los cuales fue compartido. Se detectó una diferenciación significativa entre las poblaciones Panamá­ Colombia (kst = 0.98), Panamá­Costa Rica (kst = 0.98) y Panamá­Brasil (kst = 0.72) bajo el modelo de aislamiento. Las inferencias genéticas de esta población pueden complementar la información de la capacidad de dispersión y brindar pistas importantes para comprender la ecología de Lutzom­yia longipalpisen Panamá.


Lutzomyia longipalpis is the main vector of an important neglected disease in America. The genetic div ers ity of this vector was estimated in the population collected in two geographical areas separated by up to 37 km. We analyzed the sequence CB3­PDR / N1N­PDR of 22 individuals obtaining a parameter of: h = 0.43 and π = 0.0017 (Bona), h = 0.89, π = 0.004 (The Lemon) with a genetic differentiation of kst = 0.03; p> 0.05 between them. Eight haplotypes were detected, of which it was shared. A significant differentiation was detected between the Panama­Colombia (ks t = 0.98), Panama­Costa Rica (kst = 0.98) and Panama­Brazil (kst = 0.72) populations under the isolation model. The genetic inferences of this population can complement the dispersion information and provide important clues to understand the ecology of Lutzomyia longipalpis in Panama.

ABSTRACT

Objective To investigate the efficacy of venous-arterial extracorporeal membrane oxygenation (VA-ECMO) in the treatment of refractory septic shock in children.Methods From January 2016 to December 2018,the clinical data of children with refractory septic shock (RSS) treated by VA-ECMO in Department of Critical Medicine Affiliated Children's Hospital of Shanghai Jiao Tong University were retrospectively analyzed.The patients with refractory septic shock (RSS) treated by VA-ECMO were compared with those with non-refractory septic shock (NRSS).Results There were 8 cases in the RSS-ECMO group and 6 cases in the NRSS-ECMO group.The sex,age,PRISM score,complication showed no significant difference in the two groups.The median time of ECMO in the RSS-ECMO group was 182 (141,216) h,and 5 patients were survived and were discharged from the hospital.The blood lactic acid and vasoactive drug index in the RSS-ECMO group was significantly higher than that in the NRSS-ECMO group (P<0.05 or P<0.01).The time of vasoactive drugs use and the ratio of combined continuous renal replacement therapy (CRRT) in the RSS-ECMO group were higher than those in the NRSS-ECMO group,but there was no significant difference (P > 0.05).Atter ECMO establishment,the mean invasive arterial pressure increased significantly at 6 h,and lactic acid decreased significantly at 12 h after ECMO support.SCVO2 returned to normal at 24-h ECMO therapy.Conclusions The success rate of VA-ECMO treatment in children with refractory septic shock complicated with MODS is similar to that of children with non-refractory septic shock.The relationship between ECMO and hemodynamic indexes in sepsis should be further explored.

ABSTRACT

Background: Posterior Inferior cerebellar artery (PICA) is the largest and most distal branch of Vertebral artery(VA) arising near the lower end of olive. Variations of PICA are numerous but have been infrequently reported inthis region. Occlusion or blockage of PICA may cause an infarction of the medulla oblongata which plays animportant role in circulatory and respiratory functioning. This may lead to lateral medullary syndrome, alsocalled Wallenberg syndrome. The study focuses on normal and variant anatomy of PICA in Uttar Pradesh regionof North India.Materials and Methods: The study population included 100 subjects, of either sex, who underwent Head and NeckCT Angiography in the Department of Radiodiagnosis, King George’s Medical College, Lucknow, Uttar Pradesh. Thediameters of PICAs of both sides were measured and variations encountered were noted.Results: Mean diameter of PICA was 1.76±0.64 mm on right side and 1.73±0.60 mm on left side. Mean diameter inmales was 1.75±0.62 mm and in females was 1.73±0.62 mm. Variations observed included: Unilateral aplasia ofPICA in 16% with equal prevalence on both sides; bilateral aplasia in 3% cases; hypoplasia of RPICA in10% andhypoplasia of LPICA in 6%. VA-PICA variation where the VA continued as posterior inferior cerebellar artery wasobserved in 8 subjects (4 right, 4 left).Conclusion: The knowledge of normal diameters and variations in PICA supplying the brain can be a useful guideto the surgeons and interventional radiologists for careful pre-operative planning thus helping them in avoidingpotentially life threatening complications.

ABSTRACT: The main problem in anaerobic digestion of low-protein residues is the instability caused acidity. The use of limestone at the same time as a neutralizing agent and support material is innovative because stones wear allows the slow release of the calcium carbonate thereby eliminating dispersers. Free calcium content in the system was measured in two plug flow reactors filled with vinasse at initial pH of 4.50. The proportion of 1.8 tonnes of limestone per m³ of vinasse was evaluated at the Hydraulic Retention Times (HRT) of 24, 48, 72, 96 and 120 hours, allowing stabilisation at 96 hours. The ratio of Volatile Acids/Total Alkalinity (VA/TA) ranged from 0.2 to 0.4 and the pH reached 7.0, at the HRT of 120 hours. Increasing the HRT also increased the volatile total solids (VTS) and fixed total solids (TFS) in a similar profile to the measured free calcium content, but calcium remained at the appropriate level of 100 to 250mg l-1. The proportion of limestone/vinasse was adequate to ensure stabilisation, but it is not recommended to reduce the HRT below 96 hours due to the risk of compromising the stability of the anaerobic system.


RESUMO: O maior problema na digestão anaeróbia de resíduos de baixo teor de proteína é a instabilidade por acidez. O uso de calcário ao mesmo tempo como agente neutralizador e material suporte é inovador porque permite eliminar o dosador pois o desgaste das pedras permite a liberação lenta do carbonato de cálcio. O teor de cálcio livre no sistema foi medido em dois reatores tipo Plug flow, preenchidos com vinhaça com pH inicial de 4,50. A proporção de 1,8 toneladas de calcário por m³ de vinhaça foi avaliada com os Tempos de Retenção Hidráulica (TRH) de 24, 48, 72, 96 e 120 horas, permitindo a estabilização em 96 horas. Com um TRH de 120 horas, a relação de Ácidos Voláteis/Alcalinidade (AV/AC) variou de 0,2 à 0,4 e o pH atingiu 7,0. Aumentando o TRH também aumentaram os sólidos voláteis totais (SVT) e sólidos totais fixos (STF) em um perfil semelhante ao teor de cálcio livre medido, mas mantendo níveis adequados de 100 a 250mg L-1. A proporção de calcário/vinhaça mostrou-se adequada para garantir a estabilização, mas não se recomenda a redução do TRH abaixo de 96 horas devido ao risco de comprometer a estabilidade do sistema anaeróbio.

OBJECTIVE:To investigate the effects of hydrophilic polymers on the stability of self-microemulsifying drug deliv-ery systems (SMEDDS). METHODS:Taking felodipine (FDP) as model drug,the content of FDP was determined by HPLC method. The effects of pure water,0.5% Kollidon VA64,HPMC E5,HPMC K100LV,HPMC K4M,PVP K30 solution,while 0.1%,0.5% and 1.0% HPMC E5 and Kollidon VA64 on residual content of dissolved FDP were determined in SMEDDS. RE-SULTS:The residual contents of dissolved FDP in SMEDDS placed in Kollidon VA64,HPMC E5,HPMC K100LV,PVP K30, HPMC K4M and pure water for 1 h were 92.7%,63.6%,50.2%,46.2%,36.0%and 24.0%,respectively. The order of maintain-ing the supersaturation state was Kollidon VA64>HPMC E5>HPMC K100LV>PVP K30>HPMC K4M>pure water. The residu-al contents of dissolved FDP in SMEDDS placed in 0.1%,0.5%,1% Kollidon VA64 and HPMC E5 and pure water for 1 h were 93.2%,95.1%,96.0% and 48.4%,62.1%,75.1%. CONCLUSIONS:Kollidon VA64 and HPMC E5 can significantly inhibit drug release in SMEDDS and be used as stabilizer of SMEDDS,wherein Kollidon VA64 was better.

Ischemic brain injury is a major disease which threatens human health and safety. (3, 5, 6-trimethylpyrazin-2-yl) methyl 3-methoxy-4-[(3, 5, 6-trimethylpyrazin-2-yl) methoxy] benzoate (VA-T), a newly discovered lead compound, is effective for the treatment of ischemic brain injury and its sequelae. But the poor solubility of VA-T leads to poor dissolution and limited clinical application. In order to improve the dissolution of VA-T, the pharmaceutical technology of solid dispersions was used in the present study. VA-T/polyvinylpyrrolidone (PVP) solid dispersion was prepared by the solvent method. The dissolution studies were carried out and solid state characterization was evaluated by differential scanning calorimetry (DSC), infrared spectroscopy (IR), x-ray diffraction (XRD) and scanning electron microscopy (SEM). The dissolution rate of VA-T was significantly improved by solid dispersion compared to that of the pure drug and physical mixture. The results of DSC and XRD indicated that the VA-T solid dispersion was amorphous. The IR spectra showed the possible interaction between VA-T and PVP was the formulation of hydrogen bonding. The SEM analysis demonstrated that there was no VA-T crystal observed in the solid dispersions. The ideal drug-to-PVP ratio was 1:5. In conclusion, the solid dispersion technique can be successfully used for the improvement of the dissolution profile of VA-T.

ABSTRACT

Aim To observe the effects of the transient receptor potential vanilloid receptor 1 ( TRPV1 ) antago-nist Capsazepine on the LPS-induced fever process. Methods Three doses of Capsazepine were respec-tively injected into rats’ lateral ventricles before the in-traperitoneal injection of LPS caused fever. An emitter was implanted inside the rat’ s abdominal cavity to mo-nitor the body temperature changes during the fever process. Then the changes of calcium ion in the hypo-thalamus were detected by fluorescence and the expres-sions of TRPV1 protein were detected by Western blot. Results The higher dosage of Capsazepine was injec-ted, the higher the level of LPS-induced fever was, and the lower the concentration of calcium ion in the hypothalamus as well as the TRPV1 expressions were. Conclusion Capsazepine could increase the level of fever induced by LPS via acting on the rats’ hypothala-mus. This effect might be related to the functions of TRPV1 .

Objective To investigate correlation of abdominal adipose tissue distribution and insulin resistance in T2DM. Methods A total of 128 T2DM patients were divided into two groups :obese (OG) group (n=66) and non‐obese (NOG) group (n=62). Spiral CT was used for the measurement of adipose tissue of the total area (TA) and visceral fat area (VA) at abdominal umbilical level lumbar vertebrae 4 ,5 plane in T2DM patients. Subcutaneous fat area (SA ) was calculated. General and biochemical characteristics were measured in both groups. Spearson correlation analysis showed that HOMA‐IR was positively associated with TG ,SUA ,TA ,VA ,SA ,VA/TA ,SA/TA and VA/SA. Logistic multiple regression analysis showed that TG ,SA ,TA and VA/TA were risk factors for insulin resistance in T2DM patients. Conclusion Abdominal fat distribution is closely related to IR in T2DM patients.

As ORFs I e IV do genoma do HTLV-1 codificam, respectivamente, as proteínas p12/p8 (acessória) e Tax (regulatória). p12/p8, de 99 aminoácidos, pode ser clivada em sua extremidade amino terminal gerando a proteína p8. A primeira clivagem proteolítica de p12 remove o sinal de retenção ao RE, enquanto a segunda clivagem, gera o produto de 8kDa, referido como p8. p12 localiza-se no sistema de endomembranes, residindo em RE e aparato de Golgi, enquanto p8 dirige-se para a membrana plasmática, onde é recrutada para a sinapse imunológica, através da ligação com o receptor de células T (TCR), além de participar da sinapse virológica e da formação de conduítes. A proteína Tax, por outro lado, atua como transativador transcricional do HTLV-1, sendo referida também na indução da expressão de diversos genes celulares, aumentando a proliferação e a migração das células infectadas. Na via de transporte de vesículas secretórias, vesículas produzidas como pós-Golgi são transportadas ao longo do citoesqueleto por motores celulares. A Miosina-Va, um motor não convencional, transporta diversos cargos, incluindo vesículas secretórias, vesículas sinápticas e de retículo endoplasmático. HTLV-1 ORFs I and IV encode respectively p12/p8 (accessory protein) and Tax (regulatory protein). The 99 amino acid p12 protein can be proteolytically cleaved at the amino terminus to generate the p8 protein. The first proteolytic cleavage removes the ER retention/retrieval signal at the amino terminus of p12, while the second cleavage generates the p8 protein. The p12 protein localizes to cellular endomembranes, within the ER and Golgi apparatus, while p8 traffics to lipid rafts at the cell surface and is recruited to the immunological synapse upon T-cell receptor (TCR) ligation, virological synapse and conduits. Tax on the other hand acts as viral transactivator and induces expression of many cellular genes, increasing proliferation and migration of infected cells. In secretory vesicle transport, vesicles produced as post-Golgi are moved along the cytoskeleton by motor proteins. The unconventional myosin motor, Myosin-Va, moves several cargoes including secretory vesicles, synaptic vesicles, and the endoplasmic reticulum. ABSTRACT

El tumor de Vanek o pólipo fibroide inflamatorio, se conoce como una lesión benigna, rara, de localización submucosa, no encapsulada, cuya ubicación puede darse a lo largo del tracto digestivo, siendo más común en el antro gástrico (80 %), aunque también se ha descrito en la unión gastroesofágica, duodeno, yeyuno, íleon y colon. Representa el tumor gástrico benigno menos frecuente (1-4 %), con ligero predominio en el sexo masculino y un pico de incidencia a partir de la sexta década de la vida, siendo la malignización un evento excepcional. En este reporte de caso se presentó un paciente masculino de 47 años de edad, con tumor de Vanek de localización antra, diagnosticado en el Hospital Provincial Universitario Arnaldo Milián Castro, de la ciudad de Santa Clara.


The Vanek's tumor or inflammatory fibroid polyp (IFP) is a rare benign lesion of submucosal location, not encapsulated, that may be located throughout the digestive tract, being more common in the gastric antrum (80%), although it has also been described in the gastroesophageal junction, duodenum, jejunum, ileum and colon. It is the less frequent benign gastric tumor (1-4%) with slight predominance in males and a peak incidence after the sixth decade of life, being its process of getting malignant an exceptional event. This case report presents a 47-year-old male patient with a Vanek's tumor of antral localization that was diagnosed at the Teaching Provincial Hospital Arnaldo Milian Castro, of Santa Clara.

ABSTRACT

Myosin Va functions as a processive, actin-based motor molecule highly enriched in the nervous system, which transports and/or tethers organelles, vesicles, and mRNA and protein translation machinery. Mutation of myosin Va leads to Griscelli disease that is associated with severe neurological deficits and a short life span. Despite playing a critical role in development, the expression of myosin Va in the central nervous system throughout the human life span has not been reported. To address this issue, the cerebellar expression of myosin Va from newborns to elderly humans was studied by immunohistochemistry using an affinity-purified anti-myosin Va antibody. Myosin Va was expressed at all ages from the 10th postnatal day to the 98th year of life, in molecular, Purkinje and granular cerebellar layers. Cerebellar myosin Va expression did not differ essentially in localization or intensity from childhood to old age, except during the postnatal developmental period. Structures resembling granules and climbing fibers in Purkinje cells were deeply stained. In dentate neurons, long processes were deeply stained by anti-myosin Va, as were punctate nuclear structures. During the first postnatal year, myosin Va was differentially expressed in the external granular layer (EGL). In the EGL, proliferating prospective granule cells were not stained by anti-myosin Va antibody. In contrast, premigratory granule cells in the EGL stained moderately. Granule cells exhibiting a migratory profile in the molecular layer were also moderately stained. In conclusion, neuronal myosin Va is developmentally regulated, and appears to be required for cerebellar function from early postnatal life to senescence.

ABSTRACT

Objective To analyze the present situation of intelligence of boarding school-age children of the Va nationality.Methods Multi-stage random sampling method was used to select 650 boarding school-age children from 10 primary schools in ShuangJiang county, and their level of intelligence was estimated by Raven's STANDARD progressive Matrices (SPM) .Results The level of intelligence of boarding school-age children of the Va nationality was significantly lower than the norm ( 0.05) .Conclusions The present situation of intelligence of boarding school-age children of the Va nationality is serious and should be improved as quickly as possible.

ABSTRACT

OBJECTIVE: To improve the in vitro dissolution of nimesulide by preparing nimesulide solid dispersion with hot melt extrusion (HME) technology. METHODS: Using PVP-VA64, PVP K30 or PVA-PEG (Kollicoat IR) as hydrophilic carrier, nimesulide solid dispersion was prepared by hot melt extrusion and characterized by drug dissolution, DSC, XRD and FTIR. RESULTS: Nimesulide exhibited rapid in vitro dissolution from the solid dispersion using PVP-VA64 as carrier. The cumulative release rate was 81% in 10 min, much faster than its physical mixture (only 37% in 1 h). The results of DSC and FTIR showed that nimesulide was amorphously dispersed in the carrier. CONCLUSION: Hot melt extrusion technology is suitable for preparing nimesulide-PVP-VA64 solid dispersion, which can significantly increase drug dissolution.

Introducción: la enfermedad meningocócica constituye un importante problema de salud mundial. Desde 1991 la vacuna VA-MENGOC-BC® se aplica en Cuba a los niños menores de 1 año. Objetivo: evaluar la efectividad de la vacuna VA-MENGOC-BC®. Métodos: para la evaluación poslicenciamiento de VA-MENGOC-BC® se estudiaron los lactantes con enfermedad meningocócica notificados entre 1997 y 2008. Resultados: ocurrieron 114 casos para una incidencia media anual de 7,1/100 000 lactantes. La estimación de la efectividad vacunal media resultó de 84,0 por ciento, oscilando entre 68 y 104 por ciento. La ocurrencia de enfermedad meningocócica en los no vacunados fue de 20,2 por ciento (23/114); 79,8 por ciento (91/114) en lactantes con edad de vacunación y en 75,8 por ciento (69/91) se precisó la fecha de inmunización. Tenían una sola dosis de vacuna aplicada 26,4 por ciento (24/91) y 73,6 por ciento (67/91) recibió el esquema completo (2 dosis). La enfermedad meningocócica predominó en los primeros 6 meses de edad, declinó a partir de este momento y comenzó de nuevo su ascenso a los 10 y 11 meses. Predominó la forma meníngea (89,5 por ciento); la letalidad general fue de 7 por ciento (8/114), con 4,4 por ciento para la meningococemia y 2,6 por ciento para la meningitis. Conclusiones: la efectividad de VA-MENGOC-BC® fue satisfactoria. Se sugiere realizar un análisis por un grupo de expertos sobre la necesidad de aplicar una tercera dosis.


Introduction: meningococcal disease is an important health problem worldwide. Since 1991 the vaccine VA-MENGOC-BC has been used in Cuban under one-year old infants. Objective: to evaluate the effectiveness of the vaccine VA-MENGO-BC®. Methods: for the evaluation after licensing this vaccine, all the infants affected by meningococcal disease between 1997 and 2008 were studied. Results: a total number of 114 cases were recorded. The annual average incidence was 7.1 per 100 000 infants. The mean vaccinal effectiveness for the period was 84.0 percent, ranging from 68 percent to 104 percent. The frequency of disease in unvaccinated children was 20.2 percent (23/114); 79.8 percent (91/114) within the vaccination age, but only 75.8 percent (69/91) of them had confirmed the immunization date. Only 26.4 percent (24/91) had one single dose applied whereas 73.6 percent (67/91) had completed their vaccination schedule (2 doses). The meningococcal disease prevailed in the first six months of life, declined afterwards and then started to rise again at 10 and 11 months of age. The meningeal form of clinical presentation predominated (89.5 percent); case-fatality rate was 7.0 percent (8/114), being 4,4 percent for meningococcemia and 2,6 percent for meningitis. Conclusions: the vaccine VA-MENGOC-BC® effectiveness in infants was satisfactory. It is suggested that further analysis be made by a group of experts on the use of a booster dose.

La disminución del factor de transferencia de monóxido de carbono (TLCO) y del Volumen Alveolar (VA) no es uniforme. Los informes de los equipos computadorizados entregan un valor de KCO que no toma en cuenta este detalle.Objetivo: Realizar el cálculo de KCO a través de la corrección de Stam (J Apply Physiol 1994) y compararlas con las informadas por el software de un equipo de laboratorio pulmonar computadorizado reconocido (Collins).Material y Método: Fueron incluídos pacientes consecutivos derivados al Laboratorio Pulmonar de la Unidad para realizar una prueba de TLCO por enfermedad intersticial pulmonar entre Enero y Junio de 2008. Se realizaron las pruebas según recomendacionesATS/ERS por el método de respiración única y con el método de toma de muestra de Ogilvie. Se analizó la KCO según lo informa el software del equipo Collins Plus/SQL System (1995Warren Collins), y luego se lo recalculó corregida según el cálculo de Stam (J Apply Physiol 1994). Se incluyó en este análisis si tenían <80% del volumen alveolar predicho. Resultados: Fueron evaluados 15 pacientes (media edad: 57.5 ± 12.9 años, sexo femenino 66.7%) con enfermedad intersticial.La media de VA fue 3.5±0.8 L(64.3±11.6%). La media de KCO informada a través del software fue 4.2± 1.3ml/min/mmHg/L. La media de KCO corregida fue 3.7 ± 1.2ml/min/mmHg/L (Δ 11.8±3.8, rango: 6.8%-21.1%). Se observó una relación lineal y negativa entre el %VA y el delta de KCO corregida/informada (r2= -0.99). La elección de diferentes tablas de valores normales de VA altera hasta 12% el valor de la misma, pero la KCO corregida se altera en grado mínimo (3%). Conclusiones: Cuando el VA está disminuído, se debe realizar la corrección del informe computadorizado de la KCO, porque se observa una diferencia promedio del 12%, sobreestimando la real KCO del paciente. Otros factores, como la tabla de valores predictivos de VA, influencia muy poco la corrección de KCO.


The decrease of the Transfer Factor of the Lung for Carbon Monoxide (TLCO) and the Alveolar Volume (VA) is not uniform. Software of lung computed machine informs the carbon monoxide transfer coefficient TLCO/VA (KCO) by calculating the ratio without adjusting for that assumption. Objectives: To calculate KCO using a correction by the Stam´s equation (J Apply Physiol1994), to compare the corrected KCO with the result informed by the software of Collins lung laboratory equipment, and to evaluate the impact of using different predictive tables of VA in the estimates of KCO. Materials: Consecutive patients with intersticial lung disease who attended the LungLaboratory to perform the TLCO between January and June 2008 were included in the study. TLCO was performed according to ATS/ERS recommendations by the single-breathand Ogilvie methods. KCO was calculated by the software of Collins Plus/SQL System (1995 Warren Collins), and then recalculated by Stam´s equation. Only patients withless than 80% of VA predictive value were included. Results: 15 patients with interstitial lung disease were evaluated (age: 57.53 ± 12.93 years old, female: 66.66%). The mean VA was 3.55 ±0.83 L (64.33 ±11.56%) and the mean KCO informed by software was 4.17 ±1.31 ml/min/mmHg/L. The corrected KCO was 3.76 ±1.33 ml/min/mmHg/L(Δ 11.84 ±3.84, range: 6.82%-21.1%). It was observed a negative and lineal relation between %VA and Δ KCO corrected/informed (R2= -0.99). The election of different tables of VA normal values distorts up to12% the individualvalues, but the corrected KCO is little modified (3%). Conclusion: When the VA is reduced, the correction of the KCO must be performed, to avoid on average a 12% overestimate. Other factors such as the election of VA predictive tables have little influence on the KCO correction.
